Please keep in mind that if you have Medicaid, Soonercare, O-EPIC, or an HMO that requires a referral, we will have to have the referral before setting an appointment.  You can have your Primary Care Physician fax a referral to our office at 405-691-9003.  Once the referral is received, you will be contacted for an appointment.
